There are a number of sources to check if you are looking for sample performance appraisal forms and examples to base your own forms on. Many books on performance management include examples and samples of various types of forms. Or, there are vendors that sell a variety of performance related forms. However, the Internet is probably the best source for finding a lot of free examples and samples of real forms that are used in today's working world.

Many organizations have their forms (and their policies) online, and while they aren't meant or written for the general public, they are often accessible to everyone. You are most likely to find sample and example appraisal forms from universities, government organizations, non-profits, and similar companies, although you can also find samples from for profit companies.

You should know, however, that forms you find online may not suit your particular business. It's never wise (and it's probably illegal) to copy verbatim anything you find online.
